An unshaven procedure leaves your existing hair long and hides the work underneath it. It's real, it works, and it costs more. Here's the honest version, including the part clinics leave out.
What "unshaven" actually means
Your head isn't shaved. The surgeon takes grafts from small hidden strips in the donor area — or trims them individually — so your remaining hair falls over the evidence. It's usually paired with DHI, where a Choi implanter pen places each graft between your existing hairs without pre-cutting channels. That's why the two terms get used together.
The honest timeline
Any clinic promising you walk out invisible is selling. What actually happens:
- Days 1–10 — scabs. Small crusts across the recipient area. This is the part long hair does not fully hide. Take the week off.
- Week 2 — the turn. Scabs shed, redness remains. Hair combed forward covers a great deal. Fine on a video call, fine at conversational distance.
- Weeks 3–4 — functionally discreet. Residual pinkness, but you look like a man with a haircut, not a patient. Close inspection under bright light is still possible.
- Weeks 2–8 — shock loss. Transplanted hairs shed. Expected, alarming, and meaningless. The follicles stay.
- Months 3–12 — growth. The result arrives on the same schedule as any transplant: see the recovery timeline.
Realistic plan for an office job: one week off, one week working remotely, then back in the room.
What the discretion costs
There's a real premium, and it isn't a scam — unshaven work is slower, fiddlier and harder on the surgeon. The clearest published numbers we've verified come from Civas & Akpınar in Ankara, one of the few clinics anywhere that publishes per-graft rates at all:
- Standard (men): from €1.75 per graft
- Unshaven FUE: €2.50 per graft — roughly a 40% premium
- Against a €4,500 baseline, because they run one procedure per day

Who it actually suits
- Good fit: roughly 1,500–2,500 grafts in a defined area — a receding hairline, temples, a widow's peak — with decent coverage left to hide behind.
- Poor fit: large sessions. At 4,000+ grafts the donor area needed is simply too big to conceal, and an honest surgeon will tell you to shave and accept it.
- Red flag: a clinic cheerfully offering unshaven at a very high graft count. Ask exactly how they intend to harvest that much without it showing.
The one place not to save money
Unshaven work is harder, and difficulty magnifies the gap between a skilled surgeon and a technician working through their eleventh patient of the day. Long hair combed forward hides a great deal from about week two. Realistically: a week off, then a week where you are fine at a distance and in a video call, and closer inspection is possible from about week three.
There is a real premium, because it is slower and more delicate work. Civas & Akpınar in Ankara publishes the clearest numbers we have found: €2.50 per graft for unshaven FUE against €1.75 for standard — roughly a 40% premium. Elsewhere, expect DHI to run several hundred dollars above standard FUE at the same clinic, and unshaven to sit above that again.
