Clinics market DHI and FUE like rival technologies. They're not — DHI is FUE with a different implantation step. Here's what actually changes, what it costs, and the honest answer to which one you should pick.
Same extraction, different implantation
Every modern transplant has three phases: extract grafts from the donor area, open channels in the recipient area, place the grafts. Both methods extract identically — follicular units punched out one by one. The fork comes after:
- FUE — the surgeon pre-cuts all channels (with a steel or sapphire blade), then a team places grafts into them. Two separate steps, fast at scale.
- DHI — grafts load into a spring-loaded Choi implanter pen that cuts the channel and places the graft in one motion. One step, one graft at a time.

- Pick DHI if you're rebuilding a hairline or adding density between existing hair, want to avoid a full shave, and your graft count is under ~3,000.
- Pick FUE if you need volume — crown plus front, 3,500+ grafts — or you're optimizing budget. Sapphire FUE narrows DHI's precision edge at a lower price.
- Combined plans are common — DHI for the hairline, FUE behind it. Good surgeons prescribe the method per zone, not per brochure.

DHI vs FUE questions, answered
Neither is universally better. DHI gives the surgeon finer control over angle and density — strongest for hairlines and unshaven work. FUE covers large areas faster and cheaper. Both extract grafts the same way; they differ only in how grafts are implanted. The surgeon's track record beats the acronym.

DHI is the main route to a truly unshaven transplant, because grafts go straight from extraction into the implanter pen — but it's limited to smaller sessions (usually under ~2,500 grafts) and costs more. Several clinics also offer partially shaven FUE where only the donor strip is buzzed.
FUE — usually as Sapphire FUE. Large areas need volume, and FUE handles 4,000–5,000 grafts in a session where DHI tops out around 2,500–3,500. Many surgeons combine both in one operation: DHI for the hairline, FUE for crown and mid-scalp.
