Is a Hair Transplant Permanent?
Published · every medical statement links to its source.
A hair transplant is usually permanent for the hair that is moved, but not for the hair around it. The ISHRS says an FUE transplant is permanent, and that grafts placed by an experienced surgeon should survive in most cases[1], because hair taken from the back of the head keeps its resistance to loss after it is moved[2]. Your own untransplanted hair can keep thinning[4][5], which is why many people end up planning a second procedure.
Why moved hair usually stays
The principle is called donor dominance. The ISHRS says Dr. Norman Orentreich found that hairs resistant to the hormone DHT keep their donor characteristics when transplanted into areas that are sensitive to it, and that modern hair surgery is based on this[2]. A 2008 review puts it plainly: a graft taken from an area destined to stay hair-bearing will, after an initial period of shedding, grow hair in its new site as long as it would have in the old one[3]. The ISHRS describes the donor area as the back and sides of the scalp, where hairs won't fall out during the patient's lifetime[11].
When transplanted hair does not last
- Grafts from the wrong place. The ISHRS says grafts not chosen correctly from the safe donor area may fail over time, and grafts taken from hair that was already thinning would most likely not survive[1]. A 2014 review warns that grafts taken from areas vulnerable to future loss may thin out or bald completely in their new place[8], and a 2021 review names the unsafe donor area as the main reason transplanted hair is lost[7].
- Gradual thinning in some people. A 2020 study followed 112 men in India who had a single strip (FUT) procedure for Norwood grade 4 or worse. Comparing photos at 4 years with photos at 1 year, a dermatologist outside the surgical team graded the transplanted area as unchanged in 10 of the 112 men, slightly thinner in 31, moderately thinner in 62 and greatly thinner in 9[6]. It was one centre, looking back, graded from photos without hair counts, and the authors concluded that transplanted grafts may not last permanently for everyone[6].
The hair around the transplant
Pattern hair loss keeps going. The ISHRS says hair loss is a progressive issue, and that this refers not to your transplanted hair but to the native hair you haven't lost yet[5]. The NHS says your hair might continue to thin around the transplant, and that your surgeon should plan the design so it looks natural as time passes[4]. A 2021 review says the more common reason density drops after a transplant is the loss of existing hair in the transplanted area, because a transplant does not protect the hair it was placed among[7].
Second transplants
Because the transplanted grafts are in most cases permanent, the ISHRS says further hair loss means additional transplants to fill the gaps that open between them and the receding areas[2]. Its advice is to treat any hair transplant as your first, and to keep some donor hair in reserve, because the supply is finite and non-renewable[5].
- How many procedures: in the ISHRS's 2022 survey of its members, 68% reported performing an average of one procedure per patient in 2021 to reach the desired result, against estimated averages of 3.4 in 2019 and 5 in 2016[9]. It is a survey of surgeons, not a count of patients.
- Large areas: the NHS says a large area may need 2 or more sessions on different days[4].
- Density: a 2008 review says a second sitting can be used to increase density, at least three to six months after the first[3]. A 2026 review says touch-up sessions should wait until final growth, typically around 12 months, and that expectations should extend beyond the first year because hair loss may keep evolving[10].
What permanent does not mean
The ISHRS says hair restoration can be revised over time with additional surgery, but it cannot be undone[11]. Every method leaves scars, and the donor hair used is gone for good[5]. Before you book, ask the surgeon how they would handle further loss in ten years, and how many grafts they would keep in reserve for it. If you are budgeting for a second session, the calculator below estimates a graft count.
What grafts cost where clinics publish a rate
Only 2 of the clinics we track publish a per-graft rate on their own site, too few for a middle band, and they run from $1.90 to $3.92 per graft. Read from our directory each time this page loads. We never divide a package price into a per-graft rate to fill the gap.

Questions, answered
Usually, for the hair that is moved. The ISHRS says an FUE transplant is permanent and grafts placed by an experienced surgeon should survive in most cases. Your own untransplanted hair can keep thinning, and in a 4-year photo follow-up of 112 men after strip surgery, 102 showed some thinning of the transplanted area.
Possibly. The ISHRS says that if further hair loss occurs, additional transplants will be needed, and advises treating any transplant as your first. In a 2022 ISHRS member survey, 68% of members reported an average of one procedure per patient in 2021 to reach the desired result.
It can. The ISHRS says grafts not taken from the safe donor area may fail over time, and a 2014 review says grafts from areas prone to future loss may thin or bald in their new place.
