4 Months After a Hair Transplant
Published · every medical statement links to its source.
Four months after a hair transplant, new hair is usually just beginning to show. The NHS says new hair will usually start to appear after about 4 months, and the full result should be seen after 10 to 18 months[1]. The ISHRS says hairs grow about 1 cm a month[2], so hairs that started recently are still short. Thin coverage at month 4 is expected; it is an early checkpoint, not a verdict.
What month 4 usually looks like
The resting phase is ending. The ISHRS patient brochure says the roots lie dormant for 10 to 14 weeks before the new hairs begin to grow[5], and a 2008 review says new hairs start growing about three months after the procedure[3]. By month 4 the earliest of them may have had a few weeks to grow. The ISHRS says the result takes several months at a minimum, and often a year or more[2].
Why it can still look thin
- The new hairs are short and fine. The ISHRS says that as transplanted hair matures, grows in length and increases in diameter, more volume will be realised, and that volume is proportional to the width and length of the hair[8].
- Your own hair may still be recovering. Native hair lost to shock loss usually begins to regrow around 3 months, according to a 2026 review[6]; a 2021 review says after 3 to 4 months[7].
- Untransplanted hair can keep thinning. The NHS says your hair might continue to thin around the transplant[1].
What the studies show near month 4
No study we read reports a figure for month 4. The nearest is a university hospital study of 11 men that counted grafts in small tattooed test areas: survival by graft-unit count was 57.4% at 3 months, and survival by hair count was 92.0% at 6 months[4]. It counted units at 3 months and total hairs at 6, in a small group, so it shows the direction of travel in that study, not what to expect on your own scalp.
Numbness and bumps
The ISHRS patient brochure says numbness in the donor or recipient area usually disappears within 2 to 8 months of surgery[5]. Pimple-like bumps should be settling: in one surgeon's record of 2,896 patients in India, sterile folliculitis was seen in 203 and lasted from 3 weeks to 3 months[7]. The NHS says to contact the clinic as soon as possible about severe pain or any unexpected symptoms[1].
When to judge, and what to ask
A 2008 review says it usually takes six to nine months to appreciate the result of a hair transplant[3]. A 2026 review names perceived low density and delayed regrowth in the first months among the main causes of dissatisfaction, and says touch-up procedures should only be considered once final growth is reached, typically around 12 months[6]. Regrowth timing varies from person to person. If yours worries you, send your surgeon photos and ask what they expected to see by now for your case.
What grafts cost where clinics publish a rate
Only 2 of the clinics we track publish a per-graft rate on their own site, too few for a middle band, and they run from $1.90 to $3.92 per graft. Read from our directory each time this page loads. We never divide a package price into a per-graft rate to fill the gap.
